Puppy application form

Here at Manorroy, we take great care in who with and where we home our puppies. The following short questionnaire is to provide us with some basic information about potential new owners and also to make sure you have thought about the relevant needs when considering buying a puppy.

There are no right or wrong answers, all forms will be considered depending on individual circumstances. Please answer all questions as honestly and fully as possible and write any additional information you think would be relevant on the area provided at the bottom of the form. If you are able to email pictures of your garden too that would be great. It doesnt have to look like Buckingham Palace and be too neat and tidy, just to show us it is secure and suitable for the dog and so we could maybe see any potential hazards that first time owners may not think of.
